Permit Conditions 2003 <br />Page 3 <br />3. Alcoholic beverage control permits shall be. submitted to the District no later than one <br />week prior to the event. <br />4. All appropriate copies of Certificates of Coinsurance, naming Conejo Recreation and <br />Park District, its officials, officers and employees, as additionally insured, shall be <br />provided to the Conejo Recreation and Park District prior to issuance of any permits. <br />5. CVD will obtain a special event permit from the City. <br />General <br />Annually, prior to any work being accomplished at the Conejo Creek Park site by or for the Conejo <br />Valley Days event, a formalized meeting will be held at the site to review the facilities. All <br />appropriate personnel representing the Conejo Valley Days Committee, as deemed necessary by the <br />Committee, and necessary staff from the Conejo Recreation and Park District should attend.. A <br />schedule of site preparation activities will be provided to CRPD at this meeting, and coordination <br />between those activities and the anticipated Persian. Festival on March 28, 2004 and CRPD's_ April <br />3 Z 2004 Eggstravaganza, and the City's Arbor/Earth Dann April 17, 2004 will be established. <br />GRPD -;�;1 :11- -e onsite fi;a , 7.00 11 ta0 imno ri, and the e,, went- A411 take Flute <br />If site condition problems occur over and above those enumerated in this document and are not <br />resolved, a meeting will be called between appropriate parties. If a resolution cannot be reached <br />within one week after the close of the Conejo Valley Days event, the issues will be taken to the <br />Conejo Recreation and Park District Board. At that time, the Board of Directors of the Conejo <br />Recreation and Park District will determine the resolution, and direct the remedy of same. <br />Failure to comply with the conditions as stated could result in revocation of the permit. <br />c: CVD <br />City of Thousand Oaks — Tom Hare <br />
