Annual award recipients will be invited to be recognized by the Board of Directors at <br />the Board meeting held on the third Thursday in May of each year. <br />0 Recipients will receive a personalized award. <br />2.) Athletic Achievement Award: Recognition of an individual athlete of any age, or: an entire <br />team, from any school or sports organization within CRPD District boundaries, or who resides <br />within CRPD District boundaries, who has displayed exceptional athletic achievement or <br />success in their sport. Examples may include CIF Championship teams, Little League World <br />Series Teams, or resident adults who have achieved athletic greatness. <br />L 11 <br />M�*=� <br />The CRPD Board Members may, at their discretion, select a nominee at any time, <br />or, Nomination Forms may be submitted by the general public. - <br />o Nomination Forms can be found on the CRPD website and turned in to the <br />CRPD Sports Department. <br />Award Nominees will be recommended to the CRPD Board of Directors, Athlete <br />Recognition Program Committee, for approval. <br />Multiple award winners may be recognized throughout the year, either at the time of <br />the achievement or at the discretion of the CRPD Board of Directors. <br />Award recipients will be invited to be recognized by the Board of Directors at a <br />meeting date to be determined by timeliness of the achievement or when best fits <br />into schedules. <br />0 Recipients will receive a standardized medal and a personalized certificate. <br />HILLCREST CENTER 403 WEST HILLCREST DRIVE, THOUSAND OAKS, CALIFORNIA 91360-4223 <br />(805) 495-6471 FAX: (805) 497-3199 E-MAIL: parks@crpd.org <br />
