Holt, Director <br />Chuck Huffer, Director <br />Nellie Cusworth, Director <br />GENERAL MANAGER EMERITUS <br />Tex Ward <br />CRPD has two separate awards intended to recognize outstanding performance of Conejo <br />Valley athletes: The Young Athlete Award and the Athletic Achievement Award. Each award <br />has unique requirements and nomination processes, described in detail below. <br />1.) Young Athlete Award: Recognition of a single youth athlete, under 18 years of age, from <br />any school or youth sports organization within CRPD District boundaries, who displayed <br />exceptional athletic achievement in their sport, within the school year. Examples may include, <br />but are not limited to, League MVP, All CIF selection, or Ventura County Player of the Year. <br />The CRPD Sports Department will send Nomination Forms to the Coaches and <br />Athletic Directors of high schools within District boundaries by February 1 each year. <br />Coaches or Athletic Directors of any school and youth sports organization within <br />CRPD District boundaries may also submit Nominations Forms. <br />o Nomination Forms can be found on the CRPD website and turned in to the <br />CRPD Sports Department. <br />0 Nomination Forms are due back to the CRPD Sports Department by April 1. <br />Nomination Forms will be reviewed by the CRPD Sports Advisory Council and one <br />applicant will be selected as the Award Nominee. <br />o If the CRPD Sports Advisory Council determines that more than one <br />candidate is deserving of the award, multiple recipients may be awarded. <br />Award Nominees will be recommended to the CRPD Board of Directors, Athlete <br />Recognition Program Committee, for approval. <br />0 If no applications are received, no award will be given. <br />ADMINISTRATIVE OFFICES <br />403 West Hillcrest Drive I Thousand Oaks, CA 91360-4223 <br />(805) 495-6471 1 FAX: (805) 497-31991 Email: parks@crpd.org I Website: www.crpd.org <br />
