<br />Youth Outreach Summary Report Page 2 of 4 Revised 01.2021 <br />School Report <br />Activities Conducted by Youth Outreach Staff each Month: <br />September <br />• Worked on the development of new Scope of Youth Outreach Services for the Conejo Valley Unified School District <br />• Re-connected with school counselors via phone calls and emails <br />• Outreach staff created lists of students that interacted with Outreach Workers during the 2019-2020 school year <br />• Worked on planning for the 2020 Virtual Meet & Greet event <br />• Communicated with school-based therapists through emails <br />• Supported Food Share at Conejo Creek South <br />October <br />• Continued work on the new Scope of Youth Outreach Services for the Conejo Valley Unified School District <br />• Re-connected with more school administrators and school counselors <br />• Sent school administrators and counselors lists of students who interacted with Youth Outreach Workers during <br />the 2019-2020 school year. The purpose of this was to assist school staff with identifying students that may still <br />need Youth Outreach support and services <br />• Sent revised parent/student permission forms to schools <br />• Sent Zoom link for the Virtual Meet & Greet to school representatives <br />• Continued to coordinate and plan the 2020 Virtual Meet & Greet <br />• Supported Food Share Program at Conejo Creek South <br />• Worked on a Camp Grizzley application for a TOHS student <br />November <br />• Continued recruitment process of CVUSD students that could benefit from Youth Outreach new scope of support <br />and services via emails, phone calls and Zoom meetings with school representatives <br />• Coordinated with school counselors to get free bikes, helmets, and locks to several needy students, by way of the <br />Cycles for Success program <br />• Conducted the 2020 Virtual Meet & Greet with 22 social service providers and 45 representatives from the Conejo <br />Valley Unified School District <br /> <br />Student Interest in Modified Scope of Services <br />Count of students who expressed interest in Youth Outreach services in the month of November 2020: <br />Outreach Worker Middle School High School Total Students <br />Pete Martinez 8 12 20 <br />Elizabeth Sorensen 5 10 15 <br />Dan Nalbandian 18 25 43 <br />Grand Totals 31 47 78 <br /> <br /> <br />
