Attached is the Gratitude Circle Honoree <br />Nomination Form for 2020. As a reminder, here are the criteria for the Board to use when thinking <br />about their nominees: <br />The honoree must be an individual <br />® The individual must have been involved in service to the community for at least five years. <br />• The individual should not be a currently elected or appointed CRPD official or employee. <br />• The individual should have directly or indirectly invested significant time, effort or energy to <br />improve park facilities or recreation programs which benefit Conejo Valley residents. <br />• Two honorees should be selected annually (one to three honorees have been recognized <br />annually) <br />On the form provided, please nominate up to 3 people who you believe meet the criteria.
